Subject: Revision of Ceiling Rates and guidelines for various Coronary /Vascular Stents for CGHSICS(MA) beneficiaries.

In supersession of the Office Memorandum of even No dated 12.09.2007 of the Ministry of Health & Family Welfare for the revised rates and guidelines for Coronary / Vascular Stents for reimbursement of CGHSICS(MA) beneficiaries, all DCGI approved Coronary / Vascular Stents are approved for reimbursement to CGHSICS(MA) beneficiaries as per the ceiling rates mentioned below

1, DRUG ELUTING CORONARY STENTS-

i. All DCGI and FDA approved Drug Eluting Stents – Rs. 65,000/-

ii. Ail DCGI and CE approved Drug Eluting Stents - Rs. 50000/-

iii All DCGI approved Drug Eluting Stents - – Rs 40000/-

2 BARE METAL CORONARY STENTS

i) STAINLESS STEEL STENTS – Rs.12000/-

ii) COBALT STENTS – Rs.20,000/.

a) All DCGI and FDA approved

b) Ail DCGI and CE approved – Rs. 18,000/-

c) All DCGI approved – Rs. 15,000/-

iii) COATED I OTHER STENTS – Rs. 25,000/-

3. BARE METAL VASCULAR (NON CORONARY ) STENTS

i) STAINLESS STEEL STENTS – Rs 20000/-

ii) COBALT STENTS – Rs 22,000/-

iii) NITINOL / OTHER STENTS – Rs 25,000!-

Reimbursement to beneficiaries /empaneled hospitals shall be limited to ceiling rates or as per actuals, whichever is lower.

2. Coronary Stents shall be permitted on the advice of Govt. Specialist, of which not more than two shall be of Drug Eluding Stents. Permission shall be granted as per the laid down procedure.

If more than two drug eluding stents are implanted in an empaneled hospital and no written informed consent was obtained from the beneficiary that he / she would bear the difference in cost between the DES and Bare Metal Stent, arid the hospital has charged this amount from the beneficiary, the additional amount shall be deducted from the pending bills of hospitals and shall be paid to the beneficiary

3. It is essential for the empaneled hospitals to quote the Batch number when a coronary/vascular stent of any type is implanted in the case of a CGHS/CS (MA) beneficiary and also enclose a copy of the relevant invoices pertaining to the procurement of the stents by the hospitals. In addition to this, the outer pouch of the Stent packet along with the sticker on it on which details of the stent are printed shall also be enclosed with the medical bill for claiming reimbursement from the Govt. In case of treatment from a private non-empaneled hospital, where the treatment was taken in an emergency, it is the responsibility of the beneficiary to obtain the batch number, invoice and outer pouches of the stent(s) before the submission of the medical claim to CGHSI concerned department, as the case may be.

4. The empanelled hospital shall submit a self certified undertaking that the hospital has not charged the CGHS / CS(MA) beneficiary more than the rate at which the stent has been procured by the hospital and in case of any detection and establishment that the hospital has overcharged the hospital shall be removed from the list of hospitals empaneled under CGHS without any further notice.

5. UTI-TSL, while processing the hospital bills of coronary I vascular stents shall ensure that the hospitals have enclosed copies of the relevant invoices pertaining to the procurement of the stents by the hospitals and the outer pouch of the Stent packet along with the sticker on it on which details of the stent are printed and that the prescribed rates and the guidelines have been followed, before making provisional payments to the hospitals

6. The revised rates and guidelines shall come into force from the date of issue and shall be in force for a period of one year or tilt they are revised, whichever may be earlier.

7. This issues with the concurrence of Finance Division vide Dy.No 2979 dated 20.10.2011 of the office of AS&FA Min. of H&FW

Highlights of draft Post Office Bill 2011
1. Opening of market by reducing the exclusive privilege of Central Government:

Gradual opening of the market by phase wise removal of the monopoly proposed. Exclusive privilege reduced up to 150 gm. In case of letters and up to 50 gm. in case of express mail. Couriers can carry letters even within 150 gm and 50 gm ( the reserve area) subject to them charging twice the postal rates for letters and twice the rates for express service.

2. Sunset clause:

A 15 years SUNSET clause for complete removal of exclusive privilege in express service and Parliamentary Review of exclusive privilege in case of letters is proposed.

3. Universal Service Obligation of the Central Government:

As proposed, USO is defined as the obligation of the Central Government to provide, through the Department of Posts, basic postal services at reasonable access, affordable price and with specified service parameters throughout the country. Commitment to provide postal service, delivery and access to post office 6 days of the week except holidays.

4. Definitions:

Definitions of certain ‘terms’ are proposed keeping in view the definition adopted by different countries, as also the requirement of customers i.e.
a) “postal services” means services provided by the Central Government or on its behalf and includes services related to
(i) handling of addressed letters,
(ii) handling of addressed parcels and packages,
(iii) handling of addressed press products,
(iv) handling of these articles as registered or insured mail,
(v) express services for these articles,
(vi) handling of unaddressed articles,
(vii) Value Payable Post,
(viii) money remittance
(ix) Post Office Counter Services
(x) services on behalf of any Ministry or Department of the Central or State Governments, or services on behalf of any other organization
(xi) other services not specified elsewhere;

b) “courier services” means services related to the handling of articles of mail i.e. collection, sorting, dispatch, conveyance and delivery including
(i) handling of addressed letters subject to provisions in Section 4,
(ii) handling of addressed parcels and packages,
(iii) handling of addressed press products,
(iv) express services for these articles subject to provision in Section 4
(v) handling of unaddressed articles,
c) “express services” means postal / courier services related to handling of articles of mail and expedited delivery within a clearly specified and declared time limit with confirmation of receipt and with or without end‐to‐end integration to ensure track and trace and a record of delivery ;

d) “Registered Courier” means any person registered as such under Section ‐‐‐and includes his employee or agent or assignee;

e) “Licensee” means a Registered Courier who has been given license under Section ‐‐;

5. Registration and Licensing:

The Central Government may grant registration to any person undertaking the provision of any courier service in India, who shall be called a Registered Courier. The Central Government may also grant license to any Registered Courier, who shall be called alicensee, for providing certain specific services. There is no registration fee or license fee. Licensing conditions involve adherence to quality, guarantee relief to customers in case of any deficiency in service and commitment to ensure confidentiality and security of letter. All operators will require registration for providing any type of courier service in India. But license will be required only for reserve area, USO and letter mail.

6. Registering Authority:

It is proposed that Central Government shall appoint Registering Authority, in such manner and to perform such functions, as may be prescribed.

7. Appellate Authority:

An Appellate Authority is proposed for redressal of grievances of any person aggrieved by an order of Registration Authority.

8. Setting up Extra Territorial Offices of Exchange (ETOE) and International mail Processing Centers (IMPC) abroad:

Central Govt. may establish ETOEs/IMPCs in other countries for providing international Mail Services including express and parcel services subject to arrangements with such Postal Administrations regarding terms and conditions.

AADHAR UID Card India: All you need to know :
Who doesn’t like being identified in the crowd? Remember how good it used to feel when as a kid we were told – ‘You, my child are unique’. Now with the Indian government shouting out loud about how they’re getting closer to giving each one of us a unique identification, it somehow seems to make even more sense. For those still new to the entire concept, the Government of India has appointed the Unique Identification Authority of India (UIDAI) agency to bring its grandiose AADHAAR project to reality. The main objective is to give every Indian citizen a Unique Identification (UID) card, multipurpose in nature. And we’ve jotted down a few things you may want to know about UID, right from the aadhar card application, enrollment procedure and next.
You may not want to mistake the UID for being just another ID card though because it isn’t. Every person issued the card will be getting a 12-digit unique number that’s going to be amassed in a central database. The number will be associated with basic census and biometric tidings like fingerprints, iris and a photograph. Crossing barriers of religion, caste and creed, the numbering technique intends getting rid of fake identities in all databases. The highlight of this project is that it is for every resident and is a biometric identification. What’s more, the process is completely free and there are no costs even for enrollment.
Enrollment
To get started, you will first need to visit your closest enrollment center to obtain an AADHAR UID card application form. If you are a resident of Maharashtra, you can find your enrollment center by hitting up AADHAR’s Enrollment Centre Locator. The various stages of enrolment include the enrolment agency operator receiving verification documents from the resident. Following this, the enroller will enter the resident data which is validated. Next, the applicant’s face photograph, iris impression and fingerprints of both hands along with thumbprints are captured. The enroller will then sign off with his/her fingerprint. Applicants are next given a receipt of acknowledgement and can expect their UID card to reach them in about 6 months time via post.
Documents Needed for UID Card:
For proof of name and photo identity any one among the following should do fine – Passport, PAN card, Ration card, Voter ID, Driving License, Bank ATM and credit cards with photo, Pensioner photo card, government photo ID cards, Freedom fighter photo card, photo ID issued by recognized educational institution, Arms license, CGHS / ECHS photo card and lastly an Address card with photo issued by the postal department.
As a proof of address, you may supply any among these – Passport, Bank Passbook, Bank Statement, Ration Card, Voter ID, Driving License, Insurance policy, government photo ID cards, NREGS Job Card, Arms License, Pensioner Card, Vehicle Registration Certificate, Registered Sale / Lease / Rent Agreement, Address Card having Photo issued by Department of Posts, Caste and Domicile Certificate having Photo issued by State Govt, Freedom Fighter Card, CGHS / ECHS Card, Income Tax Assessment Order. You can also furnish any of these receipts though they mustn’t be older than 3 months – Electricity, Water, Landline telephone, Property Tax, Credit Card statement. A letter signed by the Bank, a registered company or recognized educational institute on its letterhead having a photo on it will also do. Those having a certificate of address with photo issued by an MP or MLA Group A Gazetted Officer on letterhead or the Village Panchayat head or its equivalent authority for rural areas is also applicable.
A proof for Date of Birth is optional and if you so wish you may furnish a Birth Certificate, Passport, SSLC Book/Certificate or Certificate of Date of Birth issued by Group A Gazetted Officer on letterhead

Advantages UID Card:

Stepping in as a handy identification card for various services, the AADHAR UID card will help when you’re opening a bank account, booking e-tickets, buying rations etc. The fact that the card cannot be duplicated whatsoever because it will include your fingerprints and iris impressions makes it all the more significant. Forming a basic and universal means of identification, it’s going to help various Registrars and Agencies across the country eventually develop identity-based applications.
Add to this the fact that once residents have enrolled for AADHAR, they can rest assured of not facing repeated Know Your Customer (KYC) checks by service providers. Residents not having sufficient identification documents wouldn’t be denied services. Whether it’s creating a bank account or obtaining a passport, the need to prove your identity time and again via documents will be a thing of the past.
The idea behind the AADHAR project is also to empower underprivileged and poor residents gain a clear proof of identity. This will help them reach out to services which were until now impossible like a formal banking system. Alongside, they’re also going to be able to avail of a range of other Government and private sector services. Migrants too get a mobility of identity as the card transforms into identity verification through one source.
In conclusion:
Being identified does mean a lot to most of us and the AADHAR UID card does look like accessibility to services is only going to get better. LATEST DEVELOPMENT ON STRIKE

Latest developments on some strike charters are finished hereunder for the consumption of our members.

● Orders on officiating pay in higher posts – ordered are placed for signing will be released within two days.
● Orders to recommend grant and relief up to 3 days EL to Head Postmasters – will be released tomorrow or day after tomorrow.
● Order to bring staff on Sundays & holidays – orders issued last week itself. Copy could not be obtained today. Will be exhibited after its receipt.
● Revision of Recruitment Rules to Group D – Draft has been approved and will be notified by amendments to recruitment rules.
● Two more chances to appear LGO exam for those exhausted six chances – instead of two more chances, the file has been placed for enhancement of six chances to eight chances. Since the explained the urgency for its implementation in the notified LGO exam, it is assured to released the orders before the end of this month.
● It is informed that the revision of postman recruitment Rules for Postmen will be finalized by considering the number of candidates passed in the proposed postmen examination.
● Orders for dropping the confirmation examination –The draft on orders has been placed for approval.
● Revised Recruitment Rules for Postal Assistants – As per the RR 2002, there are 49 Categories eligible to appear the exam and most of the categories are not in existence. Now the existence of the category is being processed and the recruitment rules will be finalized.
(i) All the outsider candidates' secured 60% marks will be allowed to appear the PA recruitment examination.
(ii) Unfilled LGO vacancies will be thrown open to GDS without any mark condition.
● Revised Recruitment Rules for HSG I – The Staff Selection Commission have raised some queries which have been replied. It is expected that the HSG I Recruitment Rules will be returned from SSC within a week or fortnight, 100% HSG I posts for general line has been already incorporated in the revised recruitment rules.
● Postmaster's cadre Grade III will be filled up regularly only after the approval of HSG-I Recruitment Rules since the seniority to the existing HSG-I Officials on adhoc basis shall be given seniority in the newly formed cadre.
● Appearing PH candidates for IPO exam – now the department has approved for allotment of vacancies for PH candidates in IPO exam and the file will be sent to social welfare Department for information and formal approval.
● MACP fixation on acquiring TBOP/BCR – orders have been issued.
● Cadre Restructuring Committee – we could not meet the concerned Director (SR) informed that it will be convened shortly.
● Separate cadre for PO & RMS Accountants, will be sorted out in the committee.
● We requested to expedite the release of tenure posting conditions imposed on the single handed SPMs.
● Cash handling point to GDS – Proposal for formation of a committee has been initiated to review and implement the decision.
● Protection of TRCA – File has been submitted to Finance (IFW)
● Grant of Special Allowance to all irrespective of MACP I, II, or III – the IFW sought linking of old files and files have been submitted for approval.
● Treasury Allowance – File has been submitted to IFW.
● Director (SR) told that he is processing the case of allotment of staff quarter with reference to old orders related to post attached quarters and will finalise the shortly.
In respect of other issues, we could not take follow up action today and will process further after returning from Lucknow Federal Executive (i.e.) on 28.07.2011.
